Minimally Invasive Foraminotomy of the Cervical Spine: Improving Technique and Expanding Indications

چکیده
T rise of minimally invasive techniques has returned the posterior cervical foraminotomy to the armamentarium of the spine surgeon. The morbidity associated with a midline exposure, especially with the complex insertion patterns of the posterior cervical musculature, has been difficult to reconcile with the well-tolerated anterior cervical approach. However, the development of minimally invasive techniques for posterior cervical foraminotomies not only has reversed the decreasing trend of posterior cervical approaches, but also has steadily expanded our capacity to address a variety of pathologies in the posterior cervical spine. In this report, we describe the minimally invasive technique for posterior cervical foraminotomy and laminectomy. The application of this technique in three distinct clinical scenarios is presented.

STUDY DESIGN This technique article describes accomplishing multilevel posterior cervical decompression and lateral mass screw placement through a tubular retraction system. OBJECTIVE Multilevel foraminotomy and instrumented fusion using lateral mass screw fixation can be achieved through a minimally invasive technique using specialized retractors and intraoperative fluoroscopic imaging. The aim of this paper is the report of the long-term results of a prospective study on spinal cervical lamino-foraminotomy via posterior route for the surgical treatment of cervical radiculopathies due to spondylodiscoarthrosis. The goal of the described surgical procedure is the bony decompression of the involved root leaving the intervertebral protruded or herniated discs intact. Indication, ...

BACKGROUND Minimally invasive spine surgery limits surgical trauma and avoids traditional open surgery so in the majority of cases, recovery is much quicker and patients have less pain after surgery. AIM The authors describe an endoscopic approach to anterior cervical foraminotomy (ACF) by Destandau's method. MATERIAL AND METHODS Anterior cervical foraminotomy by Destandau's method is carri...
متن کاملMinimally invasive fully endoscopic two-level posterior cervical foraminotomy: technical note.
Posterior cervical foraminotomy is an effective surgical treatment method for relieving radicular symptoms that result from cervical nerve root compression. Minimally invasive techniques and tubular retractor systems are available to minimize tissue retraction, but minimally invasive approaches can carry with them the surgical challenge of trying to pass instruments through a long narrow retrac...
متن کاملMinimally invasive tubular access for posterior cervical foraminotomy with three-dimensional microscopic visualization and localization with anterior/posterior imaging.
BACKGROUND CONTEXT Posterior cervical foraminotomy allows decompression of the nerve root with preservation of motion. A previously described endoscopic technique utilizes minimally invasive muscle splitting with routine outpatient discharge. PURPOSE The approach allows a modified tubular retraction system to be used with three-dimensional visualization and anterior/posterior fluoroscopic ima...
